Station 01 / Note

The note is the unit of work.

A clinician finishes a visit and then spends a second shift writing it up. The note feeds billing, handover, audit and the next visit. It is also where most errors hide, because it is written late and from memory.

Station 02 / Front door

Triage is a conversation before it is a prediction.

People contact a service in their own words, in their own language, often frightened. What they say first decides who sees them and when. Most failures at the front door are failures of listening and routing, not of clinical knowledge.

Station 03 / Diary

The schedule is a hidden capacity.

Empty appointment slots, double-booked rooms, missed follow-ups and a waiting list that nobody can see are the same problem seen from four sides. Capacity is rarely missing. It is trapped in rules nobody wrote down.

Station 04 / Handoff

A patient is the only person who sees the whole pathway.

Care crosses clinics, labs, pharmacies and homes. Each holds a piece. Handoffs lose context, and the person at the centre becomes the courier of their own record.

Station 05 / Evidence

Cite it or do not say it.

Clinical teams are asked for answers from a literature that grows faster than anyone can read, and the sources vary from strong trials to weak reports. A fluent summary that hides that variation is worse than no summary.
